• Responsibilities: Provides professional nursing care for adults, prenatal and pediatric patients. Includes intake, triage, patient assessment, history, vital signs, injections and outpatient procedures; performs vein puncture, EKG administration when needed; and patient education. Includes case management. Maintains communication with patients concerning treatment progress and diagnostic reports. Coordinates care with other health services team as appropriate. Identifies and coordinates referrals to community resources in regards to patient needs. Documents all professional activities, contacts and observations promptly and appropriately. Attends internal and community meetings as assigned and participates in Quality Improvement activities as assigned. Participates in case management activities. Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients. Occasional travel required. Other duties or special projects as assigned. Successful candidates must possess a high school diploma and a valid license as a practical nurse in the state of Ohio. Two years experience in ambulatory care and public health setting preferred.
