The Hospice Chaplain provides spiritual care to hospice patients and their families, coordinating with the interdisciplinary team. Responsibilities include assessing spiritual needs, developing care plans, and documenting interactions.
Candidates should be ordained or licensed by a recognized denomination, with a master's in divinity preferred. Strong interpersonal communication skills and a commitment to palliative care and hospice concepts are essential.
Provision of spiritual care to hospice patients/families in coordination with the interdisciplinary team (IDG) that is consistent with the goals, definitions, philosophy, and purpose of HIA Hospice. This PRN position would provide services in Baudette, Warroad and surrounding communities. Key Responsibilities Assess the patient’s/family’s need for spiritual care or spiritual care-related services at time of admission and on an ongoing basis. Develops a written plan of care in cooperation with the patient/family and the hospice IDG that identifies needs, outcomes, and interventions for each patient/family Provides chaplaincy visits/services to patient/family as determined in the plan of care. Accurately documents all patient/family contacts and interventions in a timely manner. Provides accurate, efficient patient status reports to the team in IDGs, and in periodic updates. Facilitates communication and collaboration with the pastoral community in the office’s service area as appropriate to ensure continuity of care and enhanced awareness of hospice care. Adheres to the stipulations of contracts with other health care providers when providing hospice services in hospitals, long-term care, and assisted living facilities. Complies with the personnel policies of the agency. Complies with all policies and procedures specified in the HIPAA compliance plan. Seeks opportunities for professional development. Attends educational training programs. Adheres to the values espoused by the agency. Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy. Attends and participates in meetings related to the position of chaplain, including IDGs, family support services department meetings, and staff meetings. QualificationsQualifications Education: Ordained, ordered, or licensed by recognized denomination or equivalent experience; master’s in divinity strongly preferred. Clinical Pastoral Education (CPE) desirable.
