Case Managers facilitate direct patient care and skilled nursing visits, performing tasks such as medication management, wound care, vital sign monitoring, and patient education according to the established plan of care. Duties include completing all patient care tasks, following and reevaluating the plan of care, working scheduled shifts, and completing clinical nurse notes.
Applicants must hold a current license as a Licensed Practical Nurse or Registered Nurse from the Virginia Board of Nursing and must have prior experience working in a home care agency with direct patient care experience. The role seeks dedicated and dependable individuals to manage patient care.
