Case Managers facilitate direct patient care and skilled nursing visits, performing tasks such as medication management, wound care, vital sign monitoring, and patient education according to the established plan of care. They are responsible for completing patient care duties, following and reevaluating the plan of care, working scheduled shifts, and completing all required clinical nurse notes.
Candidates must hold a current Licensed Practical Nurse or Registered Nurse license from the Virginia Board of Nursing and possess experience working in a home care agency with direct patient care experience. The role requires dedication and dependability to manage patient care effectively.
