The Physical Therapist will provide comprehensive skilled physical therapy services in patients' homes as ordered by the physician, focusing on compassionate care and positive patient outcomes. Responsibilities include monitoring patient progress, performing necessary assessments, developing individualized care plans, and educating patients and families on recovery and equipment use.
Candidates must possess a current Physical Therapist license in the state of employment and are required to have a minimum of one year of acute care experience. A valid driver's license and auto insurance are mandatory, while prior home care experience is preferred.
