The Physical Therapist Assistant provides physical therapy services, communicates patient progress to other disciplines, and directs, supervises, and instructs non-professional home health aide staff regarding personal care. Responsibilities also include submitting required documentation like visit notes and change orders, and participating in the discharge planning process.
Candidates must possess a current unencumbered State Professional License, a valid driver's license with proof of insurance, and reliable transportation. Essential abilities include making sound professional clinical judgments and demonstrating proficiency in assessing patient needs, documenting requirements, formulating individualized care plans, and executing clinical skills.
The Physical Therapist Assistant (PTA) provides physical therapy services; communicates the patient’s progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient. As a Physical Therapist Assistant, you will: Submit completed physical therapy visit notes; communication notes per agency policy on designated days as requested by Clinical Supervisor Submit change orders Participate in discharge planning process per Medicare Criteria, policy and procedure Attend staff meetings, team conferences and educational in-services, as required Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
