This position coordinates recovery, employment, education, support groups, and housing services within agency Reintegration programs, operating under intensive clinical supervision of a Qualified Health Professional (QHP). Essential duties include documenting progress, preparing discharge summaries, providing case management, monitoring residents, coordinating external services, and ensuring resident safety during assigned hours.
Minimum requirements across all levels include a High School Diploma or GED and at least one year of experience in substance use, mental health, or human services, along with a valid New York State Driver's license and reliable automobile. Specific requirements vary by level, ranging from enrollment in educational programs (Clinical Specialist I) to holding CASAC-Trainee or full CASAC/QHP certification (Clinical Specialist II and III).

Documents individual progress in accordance with agency policy and procedure. Prepares discharge summaries in accordance with agency policy and procedure. Provides case management, including enhancement of adult daily living skills. Meets with residents regularly to monitor progress. Makes referrals, coordinates services and maintains regular ongoing consultations with other providers, legal agencies, community organizations etc. to ensure smooth transitions and minimizes fragmentation of services Helps the resident to identify and link with community agencies, resources and supports. Coordinates educational and/or vocational skill development. Organizes and maintains case records. Assists with the admission process and orientation of new residents as needed. Ensures the safety of all residents during assigned hours. Responds to all immediate social or housekeeping issues as they arise. Provides breathalyzer and toxicology screening as needed. Responds to any crises and emergencies. Completes data input into the agency database. Responds in a professional and responsible manner to incidents involving contagious diseases (i.e., HIV, Hepatitis B, TB etc.). Assists with the program’s medication self-administration policies and procedures. Assists with transportation as needed. Facilitates the resident government’s program guidelines. Provides resident education. Serves on agency committees as requested. Carries out additional duties essential to the above-stated responsibilities as assigned. Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners. Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others. Regular attendance at work location. Clinical Specialist I Minimum of HS Diploma or GED Enrolled in 350-hour educational program or completion of one-time requirements within 3 months of hire. At least one year of experience in the substance use, mental health, or human services field; and Must possess a valid New York State Driver’s license and reliable automobile
Clinical Specialist II Minimum of HS Diploma or GED CASAC-Trainee or Provisional certification Completion of a 350-hour educational program or 4000 hours of work experience. 85 hours of Section I education. Completion of OASAS one-time requirements. At least one year of experience in the substance use, mental health, or human services field; and Must possess a valid New York State Driver’s license and reliable automobile.
Clinical Specialist III Minimum of HS Diploma or GED CASAC or other QHP status certification At least one year of experience in the substance use, mental health, or human services field; and Must possess a valid New York State Driver’s license and reliable automobile
