include providing in-home physical therapy visits, completing necessary evaluations and fall risk assessments, and ensuring accurate documentation and communication of progress with the care team. The role focuses on helping seniors maintain safety and independence at home.
Candidates must possess a current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) and have at least one year of clinical experience, with home health experience being preferred. Strong documentation and communication skills, along with reliable transportation, are mandatory.
