The Care Coordinator manages a collaborative process involving assessment, planning, implementation, coordination, and evaluation of services to meet individual health needs. This role facilitates care progression, monitors patient progress, and ensures services are patient-focused, high quality, evidence-based, efficient, and cost-effective.
Candidates must be graduates of an accredited nursing school, preferably with a Baccalaureate degree, and hold current RN licensure from the Louisiana State Board of Nursing. Preferred experience includes 3-5 years of hospital adult acute care nursing, along with current BLS certification and preferred certifications like CCM, CMAC, or ACM.
Graduate of accredited school of Nursing, Baccalaureate degree preferred. Current RN licensure by the Louisiana State Board of Nursing 3-5 years of hospital adult acute care nursing experience preferred. Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: American Heart Association and American Red Cross. The ILCOR course must include a manikin check off. CCM, CMAC, or ACM preferred. Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity. Basic understanding of Medicare, Medicaid, and Managed Care regulations. Organized and efficient in establishing priorities and performing tasks within specific timeframes. Self-motivated requiring minimal supervision. Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress. Responsible for maintaining strict confidentiality of all verbal and written information and documents. Familiarity with and appropriate use of community resources. PHYSICAL DEMANDS: Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required. Must be able to work with a moderate level of noise.
