The role also involves coordinating discharge planning and participating in developing patient care plans with other disciplines.
The qualified applicant must hold a Bachelor's or Master's degree in social work from an accredited program and possess knowledge of the case management process and the needs of geriatric patients, including funding resources. Maintaining the confidentiality of professional information is also a required duty.
