• Pay: $42.34 to $50.81 per hour RN ESSENTIAL DUTIES AND RESPONSIBILITIES The Afterhours Registered Nurse will primarily conduct afterhours home visits and provide care to patients, as delegated by the Phone Triage Nurse. Provides nursing care to meet the patient’s needs in accordance with the patient’s hospice plan of care. Cross-train on phone triage to assist Triage RN team as necessary. Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data. Establishes measurable patient goals and communicates an on-going plan of care appropriately to the interdisciplinary team (IDT). Recognizes changes in the clinical status of the patient and reports findings to the appropriate team member. Collaborates with patient/family when developing the plan of care, adjusting communication style as necessary. EDUCATION & REQUIREMENTS Must possess a current RN or LPN license in the state of Colorado Must have a minimum of one year RN/LPN experience – hospice or home health experience highly preferred Must successfully pass required background checks and drug screening Requires travel to multiple work locations
