We’re seeking a compassionate and motivated Clinical Liaison RN to join our hospice team. This role blends clinical expertise with relationship-building, helping patients and families understand hospice services and ensuring smooth transitions into care.
Key Responsibilities Complete hospice eligibility assessments in hospitals, SNFs, and community settings Educate patients, families, and referral partners on hospice philosophy and services Coordinate admissions and ensure timely, accurate documentation Build strong relationships with physicians, case managers, and discharge planners Serve as a clinical resource to referral sources and internal teams
