The Weekend LPN will complete patient visits as needed on Saturdays and Sundays, working 8- or 12-hour shifts, and will be responsible for answering calls from patients, families, and customers. Visits occur in patient homes, assisted living communities, and skilled nursing facilities.
Applicants must possess a current LVN/LPN licensure in good standing and demonstrate great clinical and communication skills, along with a passion for hospice care. Candidates must also have reliable transportation, a driver's license, car insurance, and the ability to work independently while collaborating with a patient care team.

As a Weekend LPN, you will complete patient visits as needed. Applicants must have great clinical and communication skills. The Weekend LPN is responsible for answering calls from patients, families and customers. Visits occur in patient homes including private residences, assisted living communities and skilled nursing facilities. This LPN position must also be passionate about hospice care and be a great advocate for both hospice services and Brighton Hospice.
Current Licensed Vocational/Practical Nurse (LVN/LPN) licensure (in good standing) in the state(s) of practice Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system.
