The Community Health Nurse provides in-home nursing services to assist high-risk families and newborns transitioning from the Newborn Intensive Care Unit, involving developmental screenings and education on safety and health promotion. Responsibilities include collaborating with families and providers to assess needs, develop intervention plans, and ensure family self-sufficiency regarding health issues.
Candidates must possess a Bachelor's degree in nursing and have at least three years of experience in home visiting, neonatal, pediatrics, and/or maternal-child health. Applicants are also required to have a reliable vehicle, a valid AZ driver's license, and an Arizona Fingerprint Clearance Card.

Southwest Human Development is seeking a Community Health Nurse for the High Risk Perinatal Newborn Intensive Care Program (HRPP/NICP) working in the Southwest area of Maricopa County. The HRPP/NICP provides in-home community health nursing services to assist families and their newborns during the transition from the Newborn Intensive Care Unit to their home and community. This position is part time. This position is for a nurse to do 30 visits a month.
As a Community Health Nurse, you will work in collaboration with families and other community providers, complete developmental screenings, provide support and education on development, infant and home safety and health prevention/promotion. You will attend regular team meetings, case conferences and have regularly scheduled reflective supervision time to support you in your work.
