As a care coordinator, you will conduct assessments, teach life skills, document client progress, and locate resources to help clients overcome barriers. You will also provide crisis management and collaborate with the treatment team to revise treatment plans.
A bachelor's degree is required, and if the degree is in social work, a current Licensed Bachelor’s Social Worker license in Indiana is necessary. A valid Indiana driver’s license and a driving record that meets Eskenazi Health’s requirements are also required.

Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.
The Assertive Community Treatment Team (ACT) provides intensive wraparound services to adults with serious mental illness. Clients working with this team have commonly experienced recent hospitalization, incarceration, crisis services, housing instability, or a combination of these. This team is primarily community-based.
As a care coordinator, you can expect to complete case manager functions like conducting assessments, teaching life skills, documenting client progress, and locating resources to remove any barriers clients might be facing.
As a care coordinator on our ACT team, you’ll spend most of your time in the community meeting clients where they are, including in their homes, residential facilities, boarding homes, and other community locations. You will support adults living with severe mental illness, with a focus on individuals who have recently been hospitalized or have complex needs that are best addressed through intensive community-based care.
