The Licensed Practical Nurse (LPN) will support case management staff by assisting with patient assessments, monitoring, and performing medication set-ups and administration. This role also involves documenting patient status and educating patients and caregivers on care plans.
Candidates must possess an active LPN license in good standing with the Indiana State Board of Nursing and comply with all Indiana licensing requirements. Strong skills in communication, organization, and teamwork are necessary, with experience in home health or case management being preferred.

Assist with patient assessments and monitoring Collaborate with case managers and care team members Perform medication set-ups (med-set ups) and administer medications as ordered Document patient status and report changes Educate patients and caregivers on care plans and medications
Active LPN license in good standing with the Indiana State Board of Nursing Compliance with Indiana licensing requirements (IPLA), including renewal and scope of practice Strong communication, organization, and teamwork skills Experience in home health or case management preferred
