Core duties & responsibilities
Ensure that all PACT referrals are processed within the 5-day standard established by ODMHSAS.
Review PACT referral log and meet with clients during inpatient hospitalization for pre-admission screening. When possible obtain clinical records and consult with inpatient care providers for collateral information, to formulate a comprehensive case conceptualization.
Actively outreach high risk clients identified by FCS internal risk stratification reports or ODMHSAS MIN lists and attempt to engage appropriate candidates in PACT services.
Serve as a point of contact for other Crisis Care Center and FCS outpatient programs, providing clinical consultation for potential referrals to PACT teams.
Collaborate with PACT leadership and CCC staff to ensure high-risk clients transition to the most appropriate level of outpatient care, following discharge from inpatient hospitalization.
Assist PACT Leadership with completing admissions to PACT Programs.
Maintain data records on the current census, as well as monitor any flow trends with admissions and/or discharges. Data will be reported up to Senior Program Director as program needs require.
Identify and actively work to overcome any barriers that may delay admission to PACT program.
Help provide education to FCS clinical staff and community partners, regarding admission criteria, referral process and scope of services.

When there are limited openings available on existing PACT teams, this position will collaborate with PACT leadership and triage existing referrals to determine the order of admission. Interim services will be coordinated and communication maintained to ensure that clients who are appropriate for PACT services are provided admission at the earliest possible opportunity.
Serve as a liaison and primary point of contact for community partners, with particular emphasis on developing relationships with discharge planners, physicians, law enforcement, and court officials.
CORE
QUALIFICATIONS: A Master's degree in Social Work or its equivalent from an accredited school.
Knowledge and experience in individual, group and family therapy. Knowledge and experience in conducting mental health assessment, diagnosis and treatment planning.
Must demonstrate a clinical family systems orientation and ability to work with diverse populations with multiplicity of problems.
It is required that all Therapists who are eligible for licensing by the State of Oklahoma be licensed annually and all other Therapists seek appropriate supervision and meet standards that make them eligible for licensure.
Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.
