Nurse Practitioners and Physician Assistants will assess patients' medical and mental health needs to create proactive care plans that manage conditions and address barriers to care. Responsibilities also include prescribing medications, ordering necessary tests, monitoring treatment compliance, and ensuring compliance with all relevant clinical service regulations.
Candidates must possess an APRN or PA license and a degree from an accredited program, along with three years of clinical nursing or rehab experience, preferably in geriatric populations or Fee for Service settings. Experience with Electronic Health Records and proficiency in Microsoft Office applications are also required.
