The Referral Coordinator supports Care Coordinators by managing referrals, scheduling appointments, arranging transportation, and handling documentation for neighbors seeking healthcare, behavioral health, and social services. This role focuses on removing barriers to care and ensuring efficient service delivery through administrative and logistical support.
A high school diploma or equivalent is required, along with basic computer skills including Microsoft Office and data entry systems. Strong organizational skills and attention to detail are necessary, while prior experience in healthcare or social services is preferred.
The Referral Coordinator provides administrative and service coordination support to Care Coordinators. This role assists in connecting neighbors to services by coordinating referrals, scheduling appointments, supporting documentation, arranging transportation, and helping ensure timely follow-up with service providers. The Referral Coordinator plays a key support role in facilitating access to healthcare, behavioral health, social services, and community-based resources. Working collaboratively with care team members, the Referral Coordinator helps remove barriers to care and promotes efficient service delivery, so Care Coordinators can focus on direct engagement and case management.
Assist Care Coordinators with coordinating referrals to healthcare providers, behavioral health services, housing resources, transportation, and other community-based services. Schedule and confirm appointments for neighbors with medical providers, social service agencies, and community partners. Provide reminder calls, messages, and follow-ups to ensure neighbors attend scheduled appointments and services. Assist with documentation, data entry, and maintaining accurate service records in accordance with organizational and program requirements. Support the coordination of transportation for neighbors to medical appointments, service providers, or program-related activities. Track referrals and follow up with providers or agencies to ensure services were received and documented. Assist neighbors in completing forms, applications, and required paperwork related to services or benefits. Communicate regularly with Care Coordinators, and Community Supports staff to ensure continuity of care and timely service coordination. Maintain organized records and referral tracking systems to ensure compliance with program and documentation standards. Provide administrative and logistical support for care coordination activities, including preparing materials and assisting with follow-up communication. Support outreach and engagement efforts by helping coordinate appointments, initial contacts, and referral processes. Uphold confidentiality and compliance with HIPAA and organizational policies when handling neighbor information.
High school diploma or equivalent required. Any combination of education and experience that provides the knowledge and ability to perform the duties of the position. Basic computer skills including Microsoft Office, email, and data entry systems. Strong organizational skills and attention to detail.
