The role involves providing value-based primary care services, focusing on personalized, preventative care for senior patients, allowing providers to spend more quality time with each patient. Providers must show compassion and kindness while utilizing resources to significantly impact patient quality of life and avoid burnout.
Candidates must possess at least 2-3 years of experience in primary care or value-based care settings, specifically working with Medicare Advantage, Medicare, and Medicaid populations. A strong understanding of chronic disease management, preventative care, and quality metrics, along with an active Florida APRN license and DEA eligibility, are mandatory.
