The MDS Nurse (LVN) is responsible for coordinating the Resident Assessment Instrument (RAI) process, which includes completing an accurate Minimum Data Set (MDS) and collecting data to develop the Interdisciplinary Plan of Care. This role also involves participating in daily Medicare PPS Interdisciplinary Team Meetings and maintaining positive relationships within the healthcare team.
Candidates must possess a current, unencumbered LVN license as required by the state and ideally have over one year of direct nursing and MDS/RAP experience. The role requires the ability to utilize local and consultant resources and develop knowledge of state, federal, and regulatory requirements.

Coordinating the Resident Assessment Instrument (RAI) process Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care.
