Responsibilities: Under the direction and supervision of the BH Clinical Supervisor, the Clinical Therapist will provide evidence-based, and trauma-informed clinical services to current and new patients of the Behavioral Health Department.
Job
Duties: Conducts Assessment and Initial/Ongoing Treatment Planning for patients. Develops and documents clear clinical formulations and utilizes the DSM to properly diagnosis Assesses patient progress and modifies treatment plan per established timeframes Provides individual, group, family and/or couples therapy sessions for patients across the lifespan utilizing best practices and evidence-based models via in person or Telehealth services according to industry standards Develops and coordinates a clinical treatment plan to meet individual patient identified goals and diagnostic needs Provides coordination of care via referrals to other community agencies when necessary Collaborates with identified members of patient care teams both within Generations and in the community. Works closely with members of care team within the Behavioral Health and Medical/Dental Departments to improve health outcomes Assesses and creates plans for clinical emergencies, including risk to self and others Establishes and maintains a multi-disciplinary approach to treatment planning and service delivery, coordinating with medical providers and/or other behavioral health clinicians Completes all required documentation related to patient care in accordance with established policies and procedures Attends and is prepared for clinical supervision Attends department meetings, clinical team meetings and participates in committees as requested Participates in professional development and maintains required continuing education hours for licensing Maintains patient confidentiality and documents case records on all contacts regarding patient Shares in the Behavioral
