The Physical Therapist will provide physical therapy services based on a physician's written plan of care, initiating programs and instructing personnel or family members on patient goals. Responsibilities also include identifying patient needs for other home health services, participating in discharge planning, and submitting timely clinical progress summaries.
Candidates must possess a current New Hampshire Physical Therapist License, with a Doctor of Physical Therapy (DPT) preferred, and a minimum of two years of appropriate experience as a physical therapist, preferably in community or home health settings. Reliable transportation, valid driver's license, automobile insurance, a recent physical exam, and a two-step TB test are also mandatory.
