The RN Care Coordinator is responsible for triaging and assessing referrals for MFM consults and fetal diagnosis/treatment, coordinating direct patient care for specialized clinic patients, and ensuring communication among care teams and referring providers. This role also involves performing daily operations alongside physician leaders of the program.
Required qualifications include a Bachelor's Degree in Nursing and five years of clinical experience, with at least two years specifically in labor and delivery. Candidates must also possess knowledge of hospital/community resources, evidence-based guidelines, and demonstrate strong critical thinking and teamwork abilities.
Required Qualifications Bachelors Degree in Nursing 5 years clinical experience (2 of which must be labor and delivery) Understand hospital, community resources and resource/utilization management Have working knowledge of use of evidence-based guidelines Demonstrate critical thinking skills, problem-solving abilities, effective communication skills and time management skills. Demonstrate ability to work effectively on an interdisciplinary team. Have familiarity with computer systems and Microsoft applications. MN Registered Nurse (RN) License Basic Life Support (American Heart Assoc or Red Cross)
Preferred Qualifications: 1 year working as a care coordinator/case manager Case Management Certification
