The Advanced Practice Professional will evaluate, diagnose, and manage acute and chronic conditions across all age groups, while also providing preventive care and patient education. Key duties include conducting physical examinations, developing individualized care plans, and performing minor procedures within the scope of practice.
Candidates must possess a Master’s degree in Nursing (NP) or completion of an accredited Physician Assistant program, along with active NP or PA licensure and national certification in the state of employment. Experience in family medicine or primary care is preferred, alongside strong clinical and communication skills.
Job Title: Family Medicine Advanced Practice Professional (Nurse Practitioner / Physician Assistant)
We are seeking a skilled Family Medicine Advanced Practice Professional (NP or PA) to provide comprehensive, patient-centered primary care across the lifespan. The APP will evaluate, diagnose, and manage acute and chronic conditions, provide preventive care, and collaborate with healthcare teams to optimize patient health outcomes. Key Responsibilities: Conduct patient histories, physical examinations, and diagnostic assessments. Evaluate, diagnose, and manage acute and chronic illnesses across all age groups. Develop and implement individualized care plans, including medication management and referrals to specialists when needed. Provide preventive care, health maintenance counseling, and patient education on lifestyle, nutrition, and disease prevention. Perform procedures within scope of practice, such as immunizations, minor wound care, and screenings. Document patient encounters accurately and maintain compliance with regulatory and institutional standards. Collaborate with physicians, nursing staff, and other healthcare professionals to ensure coordinated, high-quality care. Participate in quality improvement initiatives, continuing education, and professional development.
Master’s degree in Nursing (NP) or completion of an accredited Physician Assistant program. Active NP or PA licensure in the state of employment. National certification as an NP or PA. Experience in family medicine or primary care preferred. Strong clinical assessment, decision-making, and communication skills. Work Environment: Outpatient family medicine clinic setting.
