We are seeking a dedicated Primary Care Physician to provide comprehensive, patient-centered care across the lifespan. The physician will diagnose, treat, and manage acute and chronic conditions, emphasize preventive care, and coordinate services to support long-term health and wellness. Key Responsibilities: Provide comprehensive primary care services, including preventive care, health maintenance, and management of acute and chronic conditions. Conduct patient evaluations, physical examinations, and diagnostic assessments. Develop and implement individualized treatment plans and coordinate care with specialists as needed. Provide patient education on disease prevention, lifestyle modification, and chronic disease management. Manage continuity of care through follow-up visits and care coordination. Maintain accurate, timely, and compliant medical documentation. Participate in quality improvement initiatives, population health efforts, and continuing medical education.
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution. Completion of an accredited residency in Internal Medicine or Family Medicine. Board certification or eligibility in Internal Medicine or Family Medicine. Active medical license to practice in the state of employment. Strong clinical judgment, communication skills, and commitment to patient-centered care. Work Environment: Outpatient clinic setting with potential for limited inpatient responsibilities depending on practice model. Team-based, collaborative environment with support from nursing, care coordination, and ancillary services. Must maintain state certification and/or licensure.
