The role involves developing and monitoring individual treatment and support plans while coordinating the Person Centered Planning process, including evaluating progress and satisfaction. Responsibilities also include providing counseling, training caregivers, assisting with community service access, and helping manage healthcare providers and financial resources.
Candidates must possess a Bachelor’s degree from an accredited college or university with a major in a human services field, adhering to Medicaid Provider Manual Guidelines. A valid Michigan driver's license is also required for this community-based role.
