The Intake Assessment Coordinator is responsible for being able to perform all aspects of Palm Point Behavioral Hospital’s intake and admission process for both Inpatient and Outpatient Servives, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual’s mental health status; making patient safety and level of care recommendations to the PPBH physician; identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information in Med Series 4. Job Duties/Responsibilities: Responds to telephone inquiries Knows PPBH programs and is able to answer general program information Determines initial clinical and financial eligbility Conducts psychosocial evaluation on each assigned admission, gathers data Coordinates with the treatment team, family and patients
Candidates must possess a Master's Degree from an accredited college or university in social work or mental health counseling. A minimum of two years of experience in the mental health field is preferred, along with strong communication and teamwork abilities.
