The role involves collaborating with an interdisciplinary team to provide expert symptom management and conduct goals-of-care and holistic, patient-centered care discussions for patients facing serious illness across inpatient and/or outpatient settings. The Advanced Practice Provider will make a meaningful impact by supporting patients and families through complex medical situations.
Candidates must be a graduate from an accredited Nurse Practitioner or Physician Assistant Program and must possess active PA Certification by the NCCPA and a Connecticut State Physician Assistant License, along with ACLS/BLS certification and a DEA License. Clinical working experience in the specialty is preferred but not required.
