Pay

$42,000 - $49,000 base pay per year

Expected Hours

37.5 per week

Schedule

  • Monday to Friday, Day Shift
  • On-call responsibilities (rotating basis with standby rate/overtime)

As a Mental Health Case Manager, you will work directly with community members to help them overcome barriers to recovery. Your role will involve locating, coordinating, and monitoring vital services and supports for individuals with mental illness.

What You’ll Do

As a Mental Health Case Manager, you will

Inspire and motivate individuals to overcome barriers to recovery.

Conduct strengths/needs assessments and create individualized Service Plans based on client preferences.

Assist clients in accessing and securing necessary services using personal transportation as needed.

Maintain and update Service Plans regularly to reflect client progress and needs.

Build and sustain ongoing relationships with clients and their support systems to ensure effective implementation of treatment plans.

Monitor client participation and progress in services, advocating for their needs with service providers.

Facilitate access to community resources and support networks to promote daily living skills and self-determination.

Coordinate and ensure continuity of care within the human service system.

Respond to emergency situations, providing assessments and referrals as needed, including facilitating commitments to psychiatric facilities.

Maintain accurate and up-to-date case records and prepare required reports within designated time frames.

Adhere to internal and external case management standards, ensuring compliance with all requirements.

Qualifications

  • Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or a related social science; OR
  • Associate degree with at least 12 college credits in a relevant field AND two years of related experience.
  • Valid driver’s license and automobile required.
  • PA Child Abuse, PA Criminal History, and FBI clearances are mandatory.
