About the role

As a Supports Coordinator, you will facilitate the creation of person-centered support plans and coordinate the delivery of supports and services. You will also monitor the health and wellbeing of participants and ensure their needs are met through regular contact.

$40,000 - $49,000 per year

Expected Hours

37.5 per week

Schedule: Monday to Friday, Day Shift, No Weekends

What You'll Do

As a Supports Coordinator, you will

Facilitate the creation of person-centered support plans with individuals, families, and support teams.

Identify and refer community and system supports and services to meet individual needs.

Coordinate and monitor the delivery of supports and services as outlined in support plans.

Ensure the quality, type, duration, and frequency of services meet individual needs.

Monitor health and wellbeing of participants through regular contact with individuals, families, and service providers.

Identify unmet needs and document them for future planning and funding opportunities.

Inform individuals of their rights and maintain comprehensive documentation through a statewide web-based system.

Participate in ongoing training to enhance your skills and knowledge.

Qualifications

  • Bachelor’s degree with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or related social science; OR
  • Associate degree with at least 12 college credits in the above fields and two years of relevant experience.
  • Required clearances: PA Child Abuse, PA Criminal History, and FBI.
  • Valid driver’s license and reliable vehicle.
