The Waiver Case Manager will foster trusting relationships with individuals served while developing, reviewing, and monitoring individualized care plans aligned with their unique needs and goals. Responsibilities also include coordinating services, connecting clients with community resources, ensuring compliance, maintaining documentation, and providing support and education to individuals and families.
A Bachelor's degree is required in social work with an active Minnesota social work license, or a Bachelor's in special education, psychology, nursing, human services, or a related field. Candidates must also have one year of experience working with individuals with developmental disabilities or complete specific training, possess a valid driver's license, reliable vehicle, and pass required background screenings.
