The Role We’re looking for a Licensed Social Worker (LSW) to join our inpatient and residential treatment team in Woodstock. Working as an integral member of the interdisciplinary clinical team, you’ll support clients through the social, relational, and systemic dimensions of their recovery, meeting people where they’re at and helping them build the connections and resources they need to move forward. This role is a great fit for someone who is organized, compassionate, and ready to do meaningful work in a structured clinical environment. You’ll practice within the scope of your LSW license under the documented supervision of a designated LISW or LICSW to provide you with built-in support and mentorship. > > This role is based at New Life Treatment Center. What You’ll Do Conduct psychosocial assessments and social history intake documentation in support of the 245G clinical team and individualized treatment planning Provide individual and group-based supportive and skills-based social work services within your licensed scope Coordinate community resources, referrals, and supportive services tailored to each client’s recovery goals Facilitate family engagement, collateral communication, and coordination with external providers and community partners Develop and support individualized discharge plans in collaboration with the clinical team, ensuring continuity of care and connection to post-treatment services Provide co-occurring or mental health-related supportive services within the scope authorized for a mental health practitioner and under documented supervision by a designated Qualified Mental Health Professional (QMHP) Maintain accurate and timely clinical documentation Fulfill mandatory reporting obligations and uphold client confidentiality under 42
