The Case Worker develops one-on-one relationships with residents to help them maintain safe and independent lives. Key duties include maintaining case files, creating individualized service plans, and facilitating social activities.
A high school diploma is required, while an associate or bachelor's degree in social work or human services is preferred. Candidates should be bilingual in English and Spanish and have an interest in working with older adults.

The Case Worker is expected to work under the guidance of the Social Worker II. They will develop 1:1 relationships with the residents which will enable them to provide a range services to the residents which enable the residents to establish and maintain safe and independent lives in the community.
