The Occupational Therapist will provide coordinated care to clients of all age groups, planning, implementing, and evaluating care plans to restore or maintain patient well-being. Responsibilities include performing clinical assessments, documenting care plans, and facilitating effective communication among team members and clients.
Candidates must have two years of experience as an Occupational Therapist in a clinical care setting, preferably with home health experience. A current state professional license, CPR certification, and reliable transportation are also required.
We are looking for a reliable and compassionate Occupational Therapist (OT) for home health to join our team.

· Promote /exemplify the Company's mission, vision, and values at all times. · Perform clinical assessments; identifies patient needs and appropriateness of client. · Document, prepare/revises, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis. · Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA). · Assess and facilitate through direct therapeutic intervention clients' ROM, strength, coordination need for and use of adaptive equipment and environmental modifications, functional mobility, and safety issues as related to clients' ADL performance needs. · Provides effective communication to clients, their families, team members, and other health care professionals, and regular participation in Case conferences/Team Meetings. Facilitates the use of these skills by other team members to achieve desired outcomes. · Evaluate client's occupational performance in the areas of self-care, work, leisure, and rest. Assess sensory-motor neuromuscular, psycho-social, and cognitive-perceptual components of occupational behavior. · Monitors assigned cases to ensure compliance with the requirements of third-party payers. · Completes appropriate documentation in a timely manner to assure compliance with company policy. · Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency. · Meet mandatory continuing education requirements of the Agency/licensing board. #HighLC1
Experience: Two (2) years’ experience as a OT in a clinical care setting. Preferred, Home health experience. Home Health OT Requirements:· Current State professional license as an OT in any/all applicable states· Current CPR· A valid driver’s license and reliable transportation, insurance required.· Therapist skills as defined as generally accepted standards of practice.· Excellent observational and problem-solving, decision-making and report writing skills.· Excellent interpersonal skills and ability to communicate effectively.
· Bachelor of Science in Occupational Therapy from an American Physical Therapy Association approved program.
