Long Term Acute Care Registered Nurse

LTAC RN: in Indianapolis

Are you an experienced RN with LTAC or STAC experience looking for a change? We currently have both day and night shift 13 week contracts available at a local hospital. These contracts may be a perfect match for you!

As an RN, you will

Manage and coordinate individualized care plans for LTAC patients.
Assess patient conditions and monitor treatment progress.
Collaborate with physicians, therapists, and social workers.
Advocate for patients and facilitate communication among all parties.
Ensure proper documentation and compliance with standards.
Coordinate discharge planning and care transitions.
Educate patients and families on care and health management.
Optimize use of healthcare resources.
Participate in quality improvement and staff education efforts.

A few must-haves for Case Manager Nurses

Active Registered Nurse (RN) licensed with in the state of Indiana
2 years of recent and related experience, as required by facility
Must be able to work independently, yet together as part of a team
Must be able to remain calm under pressure
Proficient at taking vital signs and administering medication accordingly
Active BLS, physical, TB test, and other medical compliance as required by facility
