The Certified Home Health Aide must maintain open communication with the case management team regarding patient health and safety, performing all prescribed skills competently and submitting required documentation accurately and on time. They are responsible for establishing therapeutic relationships, encouraging patient independence, and providing emotional support, while adhering strictly to the home health aide plan of care.
Candidates must possess and maintain certification as a Home Health Aide or an approved equivalent, and satisfactorily complete a Skill Assessment Test. One year of satisfactory employment history and experience in caring for the elderly are preferred qualifications.
