The Palliative Care Nurse provides direct patient and family care through consultations for seriously ill patients not yet eligible for hospice. This involves demonstrating ethical use of evidence-based palliative medicine practices across physical, emotional, spiritual, and psycho/social domains.
Required qualifications include RN/APRN licensure in the state of practice, Nurse Practitioner/CNS with prescriptive authority, current BCLS and CPR certifications, and a minimum of 5 years of clinical nursing experience along with a driver's license and reliable transportation. Advanced practice palliative care certification is encouraged after two years.
As the Palliative Care Nurse, you will provide direct patient and family care through palliative care consultations, ensuring specialty services for patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.
Demonstrates effective and ethical use of approved evidence-based palliative medicine practices in the delivery of all patient care based on physical, emotional, spiritual, psycho/social, ethical, educational, safety and related criteria, appropriate to the ages of patients served Demonstrates knowledgeable clinical expertise by incorporating palliative care body of knowledge to formulate an effective plan of care Gathers pertinent information systematically from all sources, including medical records, family history, and caregivers, and thorough specialty physical, emotional, psychosocial, and spiritual clinical patient assessment Completes a comprehensive assessment utilizing the palliative care domains of assessment, interprets findings, conducts medication reconciliation, and formulates problem-related diagnoses, initiates plan of care utilizing therapeutic interventions, and evaluates the results of these actions for adequate outcome responses ACHPN certification encouraged after 2 years of employment or when experience requirements are met
