Care Managers coordinate and integrate care for Medicaid-enrolled youth and adults (ages 0-21 or 18 to end of life) with complex conditions like SED, SMI, HIV/AIDS, or Complex Trauma. Responsibilities include completing assessments, developing and managing the Plan of Care, coordinating services based on acuity, and ensuring adherence to treatment recommendations.
require bilingual ability and specific education/experience based on the acuity level of the population served, ranging from an Associate's degree for medium acuity SED children to a Bachelor's or RN license with two years of experience for high acuity or Health Home Plus adults. Candidates must also possess strong teamwork, leadership, communication skills, cultural sensitivity, and excellent crisis intervention abilities.
The Care Managers service those who are enrolled in Medicaid, have Two or more Chorionic Conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma; ages 0-21 (Childrens Program) & 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs.

1. Specific responsibilities
This is a hybrid office- and field-based position requiring regular travel throughout Suffolk and Nassau Counties and select boroughs of New York City. Travel may occur multiple times per week based on program needs.
The role includes scheduled in-office days, with meetings and trainings held in person at our Brooklyn or Dix Hills locations, as well as virtually as needed. While every effort is made to assign care managers to cases within their designated catchment area whenever possible, flexibility to travel for service delivery is required. Applicants must reside in New York State.
