About the role

The Case Manager will perform duties related to implementing and evaluating community health worker services, including engaging in outreach, assessing client needs, and developing individual service plans. Responsibilities also involve assisting clients with plan implementation through networking, referrals to housing and food programs, and coordinating with social services and insurance providers.

Description Under the supervision of the Regional Services Manager, performs assigned duties relating to the implementation and evaluation of community health worker services. Client Services · Engage in outreach activities at various community sites to identify clients in need of case management services. · Assess client needs, prepare appropriate individual service plans and maintain records. Assist clients in implementing the service plan by networking and referral, internally and/or to other community resources. · Conduct referral and linkage activities to housing, food programs and other customer needs. · Develop and maintain database of professional contacts. · Establish, maintain and update client case records using approved agency forms and procedures. ensure confidentiality of all records. · Provide relevant literature to clients and members of the community. · Coordinate with social services and insurance companies or other financial services to ensure ability to pay for care. · Assist with insurance and benefit applications · Advocate for clients who have legal benefit concerns Assist homeless or precariously-housed clients towards more stable housing

Reporting / Monitoring · Establish, maintain, and update client case records using approved agency forms and procedures · Ensure confidentiality of all records · Prepare monthly reports in accordance with agency policies and procedures · Maintain and update client information in E-Compass and CAREWare Hyacinth Representation / Advocacy · Attend and participate in staff meetings and supervisory conferences · Maintain working relationships with other community providers · Observe established administrative procedures · Provide written reports and maintain statistics as required · Represent the agency at community events, meetings, and conferences · Adhere to Hyacinth policies and procedures including customers confidentiality provisions Training and Certifications · Attend and participate in staff training opportunities

Requirements

Preferred Minimum Education: Bachelor's degree and one-year previous office experience. A candidate who has no degree but seems exceptionally suitable will be considered if he/she can demonstrate additional years of relevant office systems experience Other: Candidate must have excellent oral presentation and writing skills. Expressed commitment to Hyacinth philosophy Preferred Area of expertise: Knowledge of supportive services and community resources.
