About the role

The Nurse Practitioner or Physician Assistant will provide direct medical screenings and primary care services to patients, including offering clinical consultation and arranging necessary referrals to other providers. Work Schedule

This role will provide clinical support across any of our STRIDE locations. 32 hours a week | 0.8 FTE

General Purpose

In order to ensure efficient, quality health care services, the Nurse Practitioner or Physician Assistant will provide medical screenings and primary care to our patients.

Essential Duties/Position Responsibilities

Provides direct health care services to patients. Offers clinical consultation and arranges referrals to other providers and clinical staff. Reviews lab and x-ray results of other providers patient care results. Prescribes medications to patients, including counseling patients regarding the proper use and contraindications of medications as well as detect and diagnose adverse drug reactions. Evaluates medical charts to determine appropriate patient care follow up and education regarding acute and chronic illnesses. Accurately and completely charts information regarding patient interactions into the Electronic Health Record (EHR). Participate in the STRIDE after-hours telephone call system. Attends all staff meetings and all other meetings as assigned.

Education and

Experience:

Required: Master’s degree in nursing from an accredited Nurse Practitioner Program and/or Master's degree in Physician Assistant studies.

Required

Licensed or eligible for licensure in Colorado (APN or PA).

Required

Board certification.

Required

Active BLS Certification. Previous experience as a nurse practitioner/physician assistant in a clinical setting is preferred. Two years post licensing preferred. Expertise: Demonstrated knowledge in primary care specialty. Ability to interact positively and build rapport with patients, coworkers and/or external contacts. Ability to demonstrate the capability to effectively and sensitively respond to the needs and concerns of the full range of STRIDE’s diverse patient population. Ability to work independently and organize work in a manner that ensures accuracy and efficiency. Ability to collaborate with other providers and clinical staff to develop continual quality improvement procedures. Ability to handle sensitive information ethically and responsibly. Ability to protect the confidentiality of patient, employee and business information. Skill in using a variety of computer software including but not limited to the internet and MS Office Products. Knowledge of and ability to work with electronic health records (EHR) system.

We offer a competitive hourly range of $108,000 - $130,000 depending on your experience. This range reflects STRIDE’s good faith estimate of potential compensation at the time of posting. The final salary for the selected candidate will be determined based on several factors, including experience, education, budget, internal equity, specialty, and training.

Integrated Care Model: Work alongside a multidisciplinary team with robust provider support, including a 1.5 MA-to-provider ratio.
