The Intensive Case Manager will provide hands-on outreach and crisis support in emergency shelters and hotels, partnering with local agencies to quickly transition individuals into stable housing while addressing critical health and social needs. Key duties include conducting intakes, immediate service planning, intensive case management, and coordinating appointments for clients.
Candidates must possess experience in homelessness, crisis response, or intensive case management, along with strong skills in communication, organization, and problem-solving. The role requires the ability to thrive in a fast-paced, field-based environment and knowledge of community resources and housing systems.

In this fast-paced role, you’ll work closely with local partners to quickly move individuals from temporary placements into stable housing while addressing critical health and social needs.
