The Case Manager will provide ongoing interventions and services to enhance stability and independence for individuals in CS Outpatient Programs, managing a caseload of up to 50 clients. Responsibilities include maintaining required monthly contacts, completing detailed documentation promptly, and meeting productivity targets.
A Bachelor’s Degree in psychology or a related field is the minimum educational requirement, along with a valid, unrestricted Georgia Driver’s License and keyboard proficiency. Preferred qualifications include one to two years of experience working with individuals with behavioral and substance use disorders, or a Master’s degree in a related field.

(This position is located in Warner Robins, GA) The individual in this role will serve as a Case Manager of the CS Outpatient Programs. The focus of this program is to provide effective on-going interventions/ services that will enhance the individual’s ability to develop a lifestyle that promotes stability and independence in their daily lives and to help individuals identify and overcome barriers that will support stabilization and recovery within self, their families and in their community. Other responsibilities of the Case Manager include effectively managing a caseload of up to 50 individuals. Complete the minimum requirements of no less then (2) contacts per month with (1) being face to face in individuals’ home or choice of a safe community setting. Keep detailed progress notes of visits to home, community locations and phone contacts. Complete all documentation into system within 48-72 hours of services provision. Meet productivity of no less than 25 hours per work week or higher each month. Execute plans of care and interventions that are person centered according to DBHDD guidelines. Provide follow up and referral services for individuals as needed. Provide community resources and contacts to the individuals to facilitate delivery of supported housing and employment services as needed. Serve as liaison between individuals their physicians and other organizational components to ensure communication and linkage for continuity of care/services. Transporting individuals served is a requirement.

One to two years of experience working with individuals with Behavioral and Substance use Disorders. Master’s degree in a related field.
