The Community Health Worker will engage children and families to provide medical and behavioral health needs, health education, and care coordination, while also acting as a liaison with local schools regarding school-related concerns. Responsibilities include conducting needs assessments, facilitating communication between families and schools, and collaborating with community providers to maximize patient health outcomes.
A High School Diploma or Equivalent is required, though an Associate's or Bachelor's degree is strongly preferred, and certification as a Community Health Worker (or ability to obtain it within six months) is mandatory. Experience working with the childhood and adolescent population is necessary, with prior experience in behavioral health or case management being preferred.
