Summary The Hospice RN in the hospital setting acts as a resource in providing education on end-of-life care to the hospital staff, patients, families and caregivers. The Hospice RN assesses patients for hospice eligibility, assists in their discharge planning, collaborates with hospice clinicians and other caregivers in the community to ensure a smooth transition to the outpatient setting, and admits patients to hospice while in the hospital when symptom management needs require an inpatient stay.
