This role provides home-based and community-based intensive case management services to youth aged 12-20 with mental health disorders or developmental delays, supporting the development of individualized goals for family and youth stability. The Coordinator participates in monthly staff meetings and receives weekly supervision from their designated supervisor.
Candidates must possess a Bachelor's Degree in Social Work or a related field, along with knowledge of serious and persistent mental illness, substance abuse, and trauma. A minimum of two years' experience working with families and children, preferably in mental health, is required, along with the ability to work independently and flexibly, including some evenings.
Full-time, 40 hours per week, hourly position. Monday – Friday; some evening hours may be required.
Office is located in South Portland. Hybrid schedule; some in office and some remote work. *Ability to do some paperwork from home/remotely.
Bachelor’s Degree in Social Work or related field required. Knowledge of serious and persistent mental illness, substance abuse, and trauma. At least two years’ experience working with families and children, preferably within the mental health system. Ability to work independently and collaboratively required. Ability to work flexible hours including evenings required. Must be able to meet physical requirements associated with family visitation, e.g. climb stairs, etc. Computer proficiency with Microsoft Office and email is required. Must be able to successfully pass a criminal background, child protective service check & sex offender check. Must not be on the state or federal suspension and disbarment list. Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, typing and frequently lift and carry small children or material weighing up to 25 pounds. Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to agency location to complete work. A good driving record is required.
