The Care Transitions Nurse conducts hospice eligibility assessments and educates patients and families about hospice services. They act as a primary point of contact to ensure coordination and continuity of care.
Candidates must possess an active RN license and have proven hospice experience with a deep understanding of Medicare regulations. Exceptional interpersonal and organizational skills are essential for this role.
