The Licensed Practical Nurse provides skilled nursing care and preventative procedures to patients in their homes according to physician orders. They also observe patient conditions, report changes to the Registered Nurse, and coordinate care with the interdisciplinary team.
Candidates must be graduates of an approved school of professional nursing and hold a current, valid LPN license. At least one year of experience in a hospital, nursing home, or hospice setting is preferred.
A qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with location policies.
Graduate of an approved school of professional Nursing. Current licensure in good standing with the state in which the LPN will be practicing. Prefer at least one (1) year of experience in a hospital/nursing home/hospice setting.
