Position Summary

The Speech Language Pathologist (SLP) provides skilled speech therapy services to patients in their homes in accordance with physician orders and the established plan of care. The SLP evaluates and treats communication, cognitive, language, voice, and swallowing disorders while collaborating with physicians, caregivers, and the interdisciplinary care team to maximize patient outcomes.

Qualifications

  • Required
  • Master's Degree in Speech Language Pathology from an accredited program
  • Current Speech Language Pathologist license in the State of Colorado
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Current CPR certification or ability to obtain upon hire
  • Valid driver's license, reliable transportation, and automobile insurance
  • Ability to work independently and exercise sound clinical judgment
  • Preferred

  • OASIS experience
  • Two (2) or more years of Speech Therapy experience
  • Experience treating adult and geriatric populations

  • Essential job duties
  • Complete speech therapy evaluations and ongoing reassessments.
  • Develop individualized treatment plans based on physician orders and patient needs.
  • Assess and treat speech, language, communication, cognitive, voice, and swallowing disorders.
  • Evaluate patients for dysphagia and provide swallowing therapy as appropriate.
  • Establish measurable treatment goals and monitor patient progress.
  • Educate patients, families, and caregivers regarding communication strategies, swallowing precautions, cognitive support techniques, and home exercise programs.
  • Collaborate with physicians, nurses, therapists, case managers, and interdisciplinary team members to coordinate patient care.
  • Complete and submit evaluations, progress notes, reassessments, recertifications, discharge summaries, and other required clinical documentation in a timely manner.
  • Complete OASIS assessments, transfers, resumptions of care, recertifications, discharges, and significant change assessments according to agency requirements.
  • Utilize ICD-10 diagnoses and coding appropriately.
  • Communicate patient progress and changes in condition to physicians and clinical leadership.
  • Participate in interdisciplinary care conferences, peer review activities, quality assurance initiatives, utilization review, and QAPI programs.
  • Maintain compliance with Medicare, Medicaid, accreditation, federal, state, and agency regulations.
  • Observe patient confidentiality and safeguard protected health information.
  • Maintain current licensure and CPR certification.
  • Perform other duties as assigned.

Requirements

Candidates must be graduates from an accredited physical therapy program and currently licensed in the state of practice. Two years of experience is preferred, along with the ability to exercise independent judgment and adhere to agency policies.
