Job Description: Licensed Practical (Vocational) Nurse (LPN/LVN))

This is not a remote position. Must live in area serviced or be willing to relocate.

Job summary

A qualified Licensed Practical (Vocational) Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.

Qualifications

1. Graduate of an approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing. 2. Minimum of one (1) year experience in nursing, preferred. 3. Acceptance of philosophy and goals of this Agency. 4. Ability to exercise initiative and independent judgment.

Line of authority: Reports to the Clinical Manager/Nursing Supervisor/RNs

Responsibilities

  • 1.
  • Furnishes services, treatments and diagnostic and preventive procedures as assigned per the plan of care, in accordance with agency policies.
  • 2.
  • Prepares clinical and progress notes.
  • 3.
  • Assists the physician, the intermediate care provider and RN in performing specialized procedures.
  • 4.
  • Prepares equipment and materials for treatments observing aseptic technique as required.
  • 5.
  • Assists the patient in learning appropriate self-care techniques.
  • 6.
  • Understands and adheres to established policies and procedures.
  • 7.
  • Implements the nursing care plan for each patient.
  • 8.
  • Initiates preventive and rehabilitative nursing procedures as appropriate.
  • 9.
  • Observes signs and symptoms and reports to the physician and registered nurse reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
  • 10.
  • Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
  • 11.
  • Evaluates with registered nurse the effectiveness of the LPN/LVN's nursing service to the patient and family under the guidance of the registered nurse.
  • 12.
  • Maintains accurate and complete records of observations, treatments and care of patient.
  • 13.
  • Participates in medical record audit as assigned.
  • 14.
  • Attends staff meetings, patient care conferences and in-services as scheduled.
  • 15.
  • Takes on-call duty, nights, weekends and holidays as assigned.
  • 16.
  • Submits any changes in schedule to Clinical Manager/Nursing Supervisor/ Registered Nurse on a daily basis.
  • 17.
  • Participates in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologist.
  • 18.
  • Observes aseptic technique as required.
  • 19.
  • Accepts, writes and executes orders; orders are cosigned by RN.
  • 20.
  • Assures that the patient/family is involved in implementing the plan of care.
  • 21.
  • Assists in reviewing, evaluating and updating the patient plan of care as appropriate.
  • 22.
  • Administers medications and treatments as ordered.
  • 23.
  • Provides appropriate information regarding patient to other agencies/ individuals involved in patient's care as needed.
  • 24.
  • Observes confidentiality and safeguards all patient related information.
  • 25.
  • Attends staff meetings and patient care conferences as scheduled.
  • 26.
  • Completes documentation and paperwork in a timely manner as per Agency policy.
  • 27.
  • Immediately reports to Clinical Manager/Nursing Supervisor/RN any patient incidents/variances or complaints.
  • 28.
  • Demonstrates competent performance of technical skills according to established procedures before performing new skills.
  • 29.
  • Participates in peer review and QAPI activities as assigned.
  • 30.
  • Participates in utilization review of medical records as assigned.
  • 31.
  • Understands and adheres to established policies/procedures.
  • 32.
  • Adheres to Agency standards and consistently interprets and accurately performs all assigned responsibilities.
  • 33.
  • Maintains acceptable attendance status, per Agency policy.
  • 34.
  • Reports all incomplete work assignments to Nursing Supervisor/RN.
  • 35.
  • Appearance is always within Agency standards; is clean and well groomed.
  • 36.
  • Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments.
  • 37.
  • Participates in in-service programs as assigned.
  • 38.
  • Maintains clean and neat work environment.
  • 39.
  • Demonstrates sound judgment, critical thinking and decision making.
  • 40.
  • Maintains current CPR certification.
  • 41.
  • Performs other duties as assigned.

Working environment

Works indoors in Agency office and patient homes and travels to/from patient homes.

Job relationships

1. Supervised by: Clinical Manager/Nursing Supervisor/RNs

Compensation negotiable based on experience. Base hourly rate for meetings and visits paid per visit.

Requirements

Must be a graduate of an approved practical nursing school and hold a current state license. A minimum of one year of nursing experience is preferred along with current CPR certification.
